We accept most insurance plans, except Humana and Medicaid. Please contact the office to confirm that we are a provider with your insurance company/plan. We will file your insurance claim for you as a courtesy. Please note that you are responsible for all co-pays and deductibles according to your policy. If a referral is required, it is the patient’s responsibility to obtain it. However, we will provide assistance in obtaining your referral if requested.
It is customary to pay for office visits when services are rendered. Payment may be made by cash, check or credit card (Mastercard/Visa).
